What if I lost my Corrective Action form?

You should visit MVD and request a Proof of Assignment form.

Where can I get a Corrective Action form or a Proof of Assignment form?

You should visit MVD or any Express store office and ask for a Traffic Survival Proof of Assignment Form.

Are there any classes during the week?

Yes, classes throughout the week are available during certain time of the year. They will be posted on site when available.

Are breaks or meals given during the 8 hours classes?

Instructors allow 2 breaks and a lunch period. However, it is the discretion of the instructor when to allow stated breaks.

Do I need to bring anything with me during class?

You MUST bring your Corrective Action or Proof of Assignment Forms if you don't have 1 of these forms you need to contact MVD at 602-255-0072. (a minimal charge may apply). A motor vehicle record is not accepted in replacement of Proof of Assignment.

What happens if I have an emergency prior to class and can't make it?

You must call our office to re-schedule. 602.434.4211.

I signed up for the wrong location, what should I do?

If you should mistakenly sign up for the wrong class location, contact our office and we can honor your request for a change. 602.434.4211.

How do I change to a different class date?

If you wish to change your class date, please contact our office for directions. 602.434.4211.

What time should I arrive for class?

Please arrive 15 minutes prior to start time.

Why do I need to take the class?

Failure to take the class course could result into your license being suspended until you pay your fines.

Are evening classes available?

Yes, evening classes are offered. The classes are offered in two half day classes.

How long are the classes?

All classes are 8 instructional hours.

Is this class certified by Motor Vehicle?

Yes, this is a certified School and all classes are certified by MVD.

If you are found responsible for, or guilty of certain specific violations, the law may require your attendance at a Traffic Survival School (TSS) class.

These classes are presented under the authority of the Motor Vehicle Division of the Department of Transportation. They have no connection to the Defensive Driving Program of the Supreme Court. It is important to know that the assignment to a TSS class is based on the finding of responsibility by the MVD.

CLASS REGISTRATION

If you have been found guilty or committed one of these violations you will be assigned to a Traffic Survival School.

If you received 3 tickets or more in one year or if you are 18 years of age and received a moving violation, it could cost you 8 to 12 points on your driving record at any time if which would also result in mandatory attendance to TSS.
